Preparing for Your Clinic Visit

Dr. Bercik sees patients at three separate locations. Please confirm the correct location for your visit.

Please arrive 15 minutes early for your first visit.

Try to wear clothing that makes it possible for Dr. Bercik to examine the affected body part. A gown can be provided for you at your visit if needed.

When you visit Dr. Bercik, please remember to bring the following:

• Driver’s License or a valid ID
• You insurance information
• Referral Letter (if applicable)
• Relative medical information and records such as X-rays, CT scans, MRIs, etc.
• List of medications
• List of allergies
